Hospital Liaison Worker

Program Overview

The Hospital Liaison Worker is responsible for providing support services to First Nations clients who seek services in the Prairie North Health Region. This is a partnership between Prairie North Regional Health Authority and Battle River Treaty 6 Health Centre that is based on equal cost-sharing, formal supervision and collaboration, with final administration accountability remaining with BRT6HC. The Hospital Liaison Worker provides the following services:

  • Advocacy
  • Discharge Planning
  • Assistance with Forms
  • Referral
  • Attending Discharge Rounds
  • Supportive Services
  • Translation
  • Medical Transportation Assistance
  • Follow Up
  • Reassurance Visits
  • LTC Visits
